The pitch you want to reach is the second note and should not start at the same time of the first 'main' note. Then in the piano roll draw your note and then a second note to the length of the slide. Most VSTs are capable of this, just look for the Solo, Glide or Mono in the plugin and switch it on. You can get some nice effects if you use Fruity Granulizer instead of Sampler.3) Use HARMOR!
